Revelstoke Trees for Tots Application for Support

Revelstoke Trees for Tots actively fundraises in order to offer financial assistance, where possible, to families in Revelstoke whose children need to travel outside our community for medical services.

We understand that all families are financially impacted by the requirement to travel for medical services. When funds are available, families can qualify to be reimbursed for up to a maximum of $1000 in eligible expenses per family.

Once your application is reviewed and approved, please keep all of your receipts (gas, food, accommodation, parking etc). Please note that original receipts are required for reimbursement.

If you have already travelled, we can only reimburse original receipts that you have.

In order to qualify for financial assistance, the following criteria must be met:

The family must reside in Revelstoke, British Columbia
An application must be completed and approved by the Revelstoke Trees for Tots Society
Original itemized receipts are required with every expense claim. Submitted receipts will not be returned.
All submitted receipts must show a date, location, and total expenses including PST and GST.

Did you know....

Many hotels have a medical rate. Ask for the 'medical rate' when booking your stay.

I understand that funds are from community donations and fundraising and that, currently, families can qualify for up to $1000 per calendar year All expenses must have an original itemized receipt. Receipts cannot be returned. If an expense does not have a receipt, it cannot be reimbursed. Credit/Debit slips cannot be accepted... receipts must show date, location, business, GST/PST Meal receipts for the child and up to two families members can be included for reimbursement I confirm that I have reviewed other options for reducing costs, including those on the "Resources" page on this website. Reimbursement from Trees for Tots should be made after accessing all other options for reducing costs. I understand that a reimbursement request form will be completed once all receipts are available. The request will be reviewed by Revelstoke Trees for Tots Society. Amounts reimbursed will be based on funding available for eligible expenses.
